Материал и методы. Проведены поиск и изучение научной литературы с использованием баз данных PubMed, MEDLINE, ScienceDirect и eLIBRARY по ключевым словам: коронарная эндартерэктомия, коронарное шунтирование, диффузный коронарный атеросклероз. Результаты. Коронарная эндартерэктомия в сочетании с коронарным шунтированием может быть эффективной методикой с приемлемым операционным риском при тщательном отборе больных на эту процедуру. Выполнение открытой методики эндартерэктомии предпочтительнее из передней межжелудочковой артерии, закрытой – из правой коронарной артерии. В качестве пластического материала при рекон-струкции эндартерэктомированной артерии предпочтительнее использовать внутреннюю грудную артерию. Наличие деэндотелизированной поверхности в зоне реконструкции коронарной артерии требует агрессивной антитромботической терапии. Эффективно использование двойной антиагрегантной терапии либо комбинации аспирина с варфарином. Таким образом, правильный подход к выбору методики хирургических методик лечения позволит улучшить отдаленные результаты у этой категории больных.</p></abstract><trans-abstract xml:lang="en"><p>The aim of the study is to review the surgical approaches to performing coronary endarterectomy incombination with coronary artery bypass grafting. Material and methods. Search and study of scientific literature using the databases PubMed, MEDLINE, ScienceDirect and eLIBRARY.RU by the keywords “coronary endarterectomy”, “coronary bypass surgery” and “diffuse coronary atherosclerosis” is performed. Results. Coronary endarterectomy combined with coronary artery bypass grafting can be an effective technique with acceptable operational risk given a careful selection of patients for this procedure. It is preferable to perform an open endarterectomy on the left anterior descending artery and closed technique on the right coronary artery. It is favored to use the internal thoracic artery as a material for reconstruction of an artery after endarterectomy. The presence of endothelial dysfunction in the area of coronary artery reconstruction requires aggressive antithrombotic therapy. The use of double antiplatelet therapy or a combination of aspirin with warfarin is effective.
